The main thrust of any claim via the SCC is the "letter" of 2 July here.
There is sufficient claimant comment on that page to include in the complaint.
The Practice Manager knew it as a report as seen at Item 7 - "a medical report" here.
Ultimately, the complainant paid £100 for a report intended for subsequent health care professionals which would have been an embarrassment for the complainant whose social status might have been compromised inasmuch that he should have checked it before sending it.

In the event of the complainant sending the original report (or letter as Mr Hazlett termed it) to any health care professional, the complainant would have been asked to respond to "What sort of report is this? It does not make sense." See foot of this page.

Ultimately, the clinic is much better off despite only paying the complainant £100.
It will have learnt that one of its professionals is below standard in a variety of ways.
That variety is spread across this website.
Ultimately, Mr Hazlett soon knew that I did not want informality. He would not have signed emails with "Al" if that wish had registered.
Even his formal letter was signed "Regards, Alan".
